How It Works

• Secure video platform — a private, encrypted connection is used for your visit, so conversations about your health stay confidential.
• Patient portal access — visit notes, records, and messages from your care team are available through our secure Electronic Health Records Portal before and after the appointment.

When to Choose a Video Visit

• Chronic condition follow-up — ongoing management of diabetes, high cholesterol, and other chronic conditions can often be reviewed remotely using data tracked through our Chronic Disease Monitoring tools.
• Nutrition and lifestyle check-ins — progress conversations tied to your individualized nutrition and activity plan can be conducted by video.
